Specialized Forms
International Students have a wide variety of needs and requirements to keep them in good immigration status while they are pursuing their degree in the United States. The PDSO and the DSO have created these documents to assist you while you are attending classes and to keep you in good immigration status. The following forms have been created for internal use only and are to be filled out by the student and their advisor. The forms should be submitted to the PDSO or DSO by the advisor. The only form that does not require the advisor's attention/signature is the I-20 reprint form.
The forms include the following:
- Reduced Course Load (RCL) allows International Students to enroll less than full-time for a specific term.
- Annual Vacation form allows students listed as 12-month programs (5-term format) to take one term off with no consequences or effect on their immigration records. Student must have completed one full academic year before the can be granted.
- Program End Date Extension form is needed when the current program end date listed on the I-20 is not when the student will actually complete their program.
- CPT Form: If your program encourages or requires internships you will need to provide the PDSO or DSO with the CPT forms so your I-20 can be updated with the information, and you can work legally. The CPT authorization is only needed for jobs where you are receiving financial compensation. You need to have your advisor's approval prior to accepting the job/internship.
- I-20 Reprint form: If an International Student requires having their immigration paperwork (I-20) reprinted for any reason, there is a request form for that specific situation
- OPT Employment Info Update form: Once you have been approved for OPT you will need to update your employer and your address information in SEVIS. You have a limited time to make those updates and if you are unable to make those updates, we have a form for that.
- Program Completion form: This form is required for OPT request. This form proves that you are in fact graduating from LU and have completed your degree requirements. This is needed at the time of applying for OPT or before graduation.
- OPT Waiver form: This form is stating that you are responsible for the application and all of the information you provided to USCIS when you submitted the I-765 form for employment approval.
